Appointments

New Patients

All new patients require a referral from a family physician, specialist or nurse practitioner.

Our office does a pooled referral system.

Once your referral has been reviewed by one of our orthopedic surgeons you will be placed on our consult waitlist based on urgency status. When an appointment becomes available, you will be contacted by mail. If your mailing address has changed, please contact our office to update your information.

Please note, that we do not give out waitlist times. We will confirm that we received your referral.

Preparing for Appointment

Once you have been sent your first appointment date, follow the instructions below:

-Bring your health services card.

-Bring your completed patient information sheet (attached to your appointment notice).

-If your information sheet has been misplaced, you can find another copy below. The name of the surgeon will be located at the top of your appointment letter.

-If an x-ray requisition was mailed to you with the appointment information, please take it to the specified clinic (Associated Radiologists) prior to the appointment to save wait times on the day of appointment. If you are from out of town, the x-ray may be done on the day of the appointment as there is a location right next to our office.
