Before surgery
The following information is to help you prepare for your surgery.
Pre-operative instructions
Once you have been sent your surgery date by mail, please read through your package carefully. The location of your surgery and registration time will be in your surgical package. Your surgery may be cancelled if these directions are not followed.
Please call your family physician to have your pre-operative form filled out, these forms will be provided by your family physician’s office or a copy can be found below. This form must be filled out prior to your surgery and brought with you on your day of surgery.
You are required to fast after midnight the night before your surgery. This means nothing to eat, drink or smoke after midnight the night before.
If you are being discharged home the same day as your surgery, please arrange for someone to drive you home.
If you take any blood thinners, please consult your family physician or your surgeon’s office for further instructions.
If your medical conditions or medications change while awaiting surgery, please contact our office to update us.
what to bring
Completed history and physical form.
Your health card.
Regular medications.
Remove all makeup, nail polish, and jewelry.
Please refrain from bringing a large amount of cash, credit cards, or valuables. The hospital is not responsible for the loss of money or other personal possessions.
If you have insurance forms to be filled out by your surgeon, these can be done for a fee. Please DO NOT bring these forms to the hospital. Contact your surgeon’s office to have these forms filled out.
